Two Skylight Healthcare Systems Clients to Launch iCarePassport

New Programs to Support Heart Failure, Bariatric Surgery

In order to respond to the challenges of a reformed health care market, two Skylight clients have elected to maximize their use of the ACCESS system by launching iCarePassport applications to help drive improvements in their clinical, financial and operational results.  Hoag Memorial Hospital Presbyterian, Newport Beach, CA, and Shawnee Mission Medical Center, Shawnee Mission, KS, have agreed to launch iCarePassport to support patient care and clinical initiatives from pre-admission through post-discharge.

For many conditions with high readmission rates, such as congestive heart failure, the time between hospital discharge and the first doctor visit can be critical.  Hoag is launching iCarePassport for heart failure patients so that the clinical team can dynamically inform them about what's required to have a successful healthcare procedure.  The ability to make a follow-up visit, while receiving information on medications, diet, exercise, and smoking cessation programs are all part of the personalized information and interactivity that can help prevent problems early—before they result in costly readmissions.

The clinical objective is to enroll Hoag CHF patients at discharge from ACCESS and the success measure will be to reduce re-admission rate by driving follow-up appointments, ensuring patients receive comprehensive medication information, in addition to ongoing support.  Benefits of the program are:

  • Hoag-Branded Health Education Content; Support Physicians by Creating CHF Content
  • Education Pre-Admission/Post Discharge; Access to Educational Information 24x7
  • Increase Compliance to Medication Regimens and Provide Positive Feedback from Interactive Communications
  • Covers Health Education during the Entire Continuum of Care

Shawnee Mission Medical Center, Shawnee Mission, KS

Healthcare consumers are more engaged and involved in planning their healthcare than ever before—especially in the areas of planned surgeries and procedures.  They are taking a very active role in choosing their hospitals based on quality and patient satisfaction.  Shawnee Mission Medical Center (SMMC) elected to launch iCarePassport with an initial focus on bariatric surgery to support its well earned and established reputation for clinical excellence in this area.  As an accredited Bariatric Center of Excellence, SMMC has a proven program that produces patient outcomes that are better than the national averages.

Shawnee Mission Medical Center's Bariatric Surgery Program assists patients who are 100 pounds or more overweight in losing those dangerous - and sometimes life-threatening - extra pounds.  Shawnee Mission Medical Center's dedicated team of bariatric surgery professionals work each day to ensure patients receive the most comprehensive, compassionate care possible—which will now include the benefits of iCarePassport for bariatric patients and their families.  Unlike other surgical procedures, preparation can take up to a full year, and post-discharge education and support is an ongoing process.  Benefits of the program include:

  • Centralize Information into One Single, Accessible Multi-Media Repository
  • Support Patients with Timely Information through the Continuum of Care; Reduces Concerns and Eases Anxieties
  • Post Discharge, Patients Receive Alerts and Reminders to Support Them through Recovery.
  • Help Reduce Calls to Triage Nurse
  • Strengthens Physician Relationships
  • Allows for Family Participation; Accessible 24 x 7
  • Unique View into Outcomes/Metrics

iCarePassport: Connecting Hospitals, Patients, Families and Healthcare Professional for Ongoing, Personalized Care

Engaging patients through a secure, hospital branded Web site; iCarePassport allows patients, families and the entire care team to intelligently prepare for the procedure and also to provide timely post-discharge care information.  Condition or procedure specific, iCarePassport is dedicated to and personalized by patient, physician (or group), and by procedure date or event.

This ongoing contact and connection helps to encourage positive steps toward patient recovery.  Patients who are well prepared for medical procedures are often more satisfied with their results and with their overall hospital experience.  In addition to an enhanced hospital experience, improved patient care, and success in overcoming difficulties inherent in patient/physician communications, diagnosis driven content and education can facilitate improved retention of vital health and safety information.

A comprehensive personal health system that encourages ongoing patient engagement, iCarePassport also integrates methodology for tracking evidence based outcomes.  Ongoing communication and collaboration between surgical, nursing, rehabilitation, family members, and patients provides a unique opportunity to extend the benefits of the healing environment far beyond the hospital stay and can help patient recover more quickly and improve their quality of life.
