Discharge Planning
CareNavigator™ helps streamline what can often be a complex and time-consuming discharge process.
Patients can receive educational content on specific issues, such as details on medications they will be taking at home.
If required, CareNavigator can be used as part of the discharge process to provide additional information by connecting the patient with the appropriate staff member, such as a pharmacist to shed additional light on a medication.
CareNavigator helps enhance three key elements of the discharge process:
- The patient's discharge education starts at the time of admission
- The patient is prepared for the hospital to home transition
- The patient's family is oriented on the process of how to best help provide post-hospital care
CareNavigator works across the continuum of care to make sure that discharge is not perceived by the patient as an abrupt event, but a planned stage on the transitions of care.
